Personal Data Sheet (face sheet) *. One example of a commonly used "face sheet" for assisted living facilities. Although this version contains all information required by most AL providers we will modify this report to accomodate your facility requests and ALL state regulatory requirements.
Resident Emergency Notification *. This report contains all relevant resident information including medications, emergency contacts, Health Care providers and Insurance information. Our clients find this report useful to send with a resident to the hospital, physician appointment or with a family member for a visit. This report contains all the vital information for a resident and enables a use to make one change and have the information updated through out the software application.
Daily Census Report *. Current roster of all residents living with in your facility at any given time.
Occupancy Report *. Current listing of all residents living with in your facility over a period of time. This report is beneficial for the business office and others to determine move-ins, move-outs, total billable days etc.
Medication Administration Record (MARS) *. Monthly documentation report for supervising resident medications (Includes PRN meds and documentation) TARS- for treatments and other directives available as well.
Order Medication *. Available report to email or fax to a pharmacy provider to order (start) a new resident medication.
Discontinue Medication *. Available report to email or fax to a pharmacy provider to discontinue (stop) an existing medication.
Physician Orders *. Current list of all medications, treatments and diagnosis. This report can be used to meet physician notification requirments in your state (most states require either annual or bi-annual notification). This report is also available for resident to take with them on doctor appointments.
Incident Report. Example of an Accident / Incident report. This information is also printed in summary format for further review and tracking (QA program).
Vital Sign / Weight Tracking *. This report is useful to determine if a resident is experiencing any undesirable weight loss or clinical complications. This is also available as summary information for the entire facility, enabling staff to understand how many residents are at risk or require intervention.
User Defined Assessment. This is an example of an assessment created from a clients existing paperwork to an electronic format (note: scoring by points,time or charges is available to assist with billing and level of care ). This system preserves the content of your assessment(s), provides flexibility to meet changing regulatory requirements, and minimizes time and effort training staff to complete modified assessments potentially containing information not appropriate for your facility. We also offer sample assessments for use and/or modification, either way, this process is fully supported by our staff.
Daily Care/Task List. This sample report often goes by different names, ADL Log, daily log, resident activity report - This one example provides an individual summary of the services you provide the resident on any given day. Like the service plan (below) the Care list is assessment driven and contains only the information specific or unique to the resident. This is a simple and effective way to track services provided to a resident on a daily basis to ensure quality and continuity of service. We also have the ability to generate billing reports from this information to capture all services provided.
Service Plan. This Service plan (collapsed version) is assessment driven. Service plans can be created from one assessment or multiple assessments depending on the facility and the procedures in place. Another option is the ability to directly edit the service plan to reflect changes without having to complete full assessments. This Service plan is also availalbe in a detail format showing the users all potential responses making it simple to review and update as needed.
Pain Assessment. An available assessment to track and identify residents experiencing or complaining of pain. This tool can enhance your pain management program and track progress across your facilty.
Security *. AL-Insight (Assisted Living Software) has many built in security features. Our software meets HIPAA requirments and industry standards for data protection, access and back-up. Each facility can set-up an unlimited number of individuals users and enable specific permissions based on job role and responsiblity. This provides greater flexibility at the user level and restricts access to resident data from non-authorized users. This feature is fully supported by Senior Insight and is included in your monthly subscription fee.

Prospect Information *. Detailed information and contact history for each inquiry (prospective resident).
Contact Information *. Detailed information and contact history for each inquiry (Contact / Residents representative).
Follow-up Report *. Query a date range to look at detailed information regarding scheduled follow-up appointments. This report organizes your marketing efforts and provides information to facilitate appropriate resident "move-in's" and census building (for example - Query the database to preview which phone calls you need to make in the upcoming week, what tours are scheduled etc.).
Detail of Inquiries *. An ideal report to share with your team to review marketing and sales efforts, understand where your leads are coming from and determine how effective your efforts have been. This tool also enables a user to understand the volume of leads, your ability to follow-up and convert potential prospects to "move-in" status.
